A Cost-Benefit Analysis of Toothbrushing Facility in Elementary School

Abstract
Backgrounds : The aim of this study was to estimate long-term economic costs and benefits of toothbrushing facilities in primary school.

Methods : Treatment cost saved from reduced number of decayed teeth constituted main benefit and was estimated based on two models. Model one used the data on the number of decayed teeth surveyed in schools with and without toothbrushing facilities. Second model used the information on toothbrushing frequency and coefficient of association between DMFT and toothbrushing frequency. Lifelong cost for decayed teeth included treatment cost for decayed teeth, periodontal disease occurring as a sequela, and prosthetic care. Costs item included the one for establishment and management of toothbrushing facilities. Time horizon was from the time when cohorts were exposed to the toothbrushing facilities to time when they reach 80 years old.

Results : Estimated benefit from toothbrushing facilities was KRW 15,916,942,925~18,005,979,365 depending on the model, and the cumulative cost was KRW 785,849,589 The benefit-cost ratio was approximately 20.3 and 22.9.

Conclusions : Establishment of toothbrushing facilities in primary school was proved to be very cost-effective. However, more reliable database on oral health need to be developed.
